Eligibility Criterion to claim PF or Pension Fund after exiting from Channelplay
  • Completion of 60 days from Last Working Day (LWD) in the organisation
  • Settlement of Full and Final Amount

Methods to claim PF and Pension Fund
  • Without Employer Intervention
  • With Employer Intervention

To claim PF and Pension Fund without employee intervention, following form (attached here for reference) needs to be filled by the employee and should be submitted to EPFO, Jaipur, Rajasthan:
  • UAN Based Form 19
  • UAN Based Form 10C
This can only be done, if employee's UAN is activated and his/ her KYC is updated with UAN.

To claim PF and Pension Fund with the help of Employer (Channelplay Limited), following needs to be done by the employee:
  • Attached PF Withdrawal Form should be filled with all personal details completed. (For reference, sample form is also attached)
  • Include cancelled cheque with printed name, copy of bank pass book and copy of PAN Card
  • Form 15G is also suppose to be attached along with PF forms.
  • Courier all the documents together to the following address:
    • Channelplay Limited
    • Plot No 721
    • Udyog Vihar Phase V
    • Gurgaon- 122016

After the courier is received by Channelplay, below mentioned process is followed:
  • All the couriers are passed on to HR Department together
  • First level verification gets run on the form to complete the missing information
  • In case of rejection, a notification is sent to the employee on their registered e-mail id
  • In case of approval, the form is signed by the authorized signatory
  • The forms are then forwarded to the consultant to run a compliance check
  • After all the checks and verification, the forms are submitted to the PF Office in Jaipur. However, there is a limit on submission of forms to 10 in a week by EPFO Department
  • After submission of form, the acknowledgement is received
  • After the acknowledgement is received, the claim status becomes visible on the employee's online portal through www.epfindia.com
